WS5: Documentation Finalisation — Final AISDP Version The AI System Assessor prepares the final AISDP version, incorporating all operational content plus a decommissioning record. The record captures the end-of-life trigger and rationale (reason, trigger identification date, governance approval), the plan and execution log (approved plan, milestone completion dates, deviations and resolutions), deployer notification records (copies of all notifications with delivery confirmation), the technical shutdown record (endpoint deactivation dates, credential revocation confirmations, infrastructure release records, final model integrity verification), the data deletion and retention record (per-category schedule with DPO Liaison signed attestation), the post-decommission obligations register (ten-year retention expiry date, monitoring commitments, data subject rights obligations), and the registration status update (EU database updated to “no longer on the market/in service”). Module 1 is updated with system status “decommissioned” and the decommission date. Module 6 records end-of-life as a risk treatment (risk eliminated by system withdrawal). Module 7 records cessation of operational oversight and transition to post-decommission monitoring. Module 10 records the decommissioning within the QMS change log. Key outputs
