Serious Incident Reports & Register The serious incident reports archive retains every report submitted to competent authorities (initial and supplementary), together with the investigation findings, corrective actions, and authority correspondence. The serious incident register is maintained alongside, tracking all assessed events regardless of determination. Key outputs

  • Complete report archive (initial, supplementary, correspondence)
  • Register of all assessed events including near-misses
  • Investigation findings and corrective actions documented
  • Ten-year retention as Module 12 evidence
