Phase 5 validates the complete system in a production-representative environment and compiles the AISDP. The system is deployed to staging; end-to-end inference, regression, and chaos/fault injection tests are executed. Performance, fairness, and robustness metrics are computed against AISDP-declared thresholds.

The AISDP is compiled from artefacts produced during development, not written from scratch. Each module is populated from engineering artefacts. The Conformity Assessment Coordinator reviews for completeness and consistency. The internal conformity assessment (Annex VI) is conducted: QMS assessment verifies Article 17 elements, technical documentation assessment examines Articles 8–15, and consistency assessment traces from AISDP to source artefacts. Non-conformities are recorded and remediated.

The operational oversight framework is established: monitoring infrastructure configured, alerting thresholds set, escalation procedures documented, break-glass procedures tested, operator training completed. Phase 5 produces the complete AISDP, internal assessment report, Non-Conformity Register, assessment evidence register, oversight readiness confirmation, and operator training records. The AI Governance Lead signs the Declaration of Conformity once all critical non-conformities are resolved.
