Phase 5: Synthesis & Reporting

Phase 5 consolidates findings from all preceding phases into the formal Assessment Report. The assessor classifies each non-conformity by severity (critical, major, minor), reconciles findings across phases to avoid duplication, and reaches an overall assessment conclusion. Typical duration is two to three days.

The conclusion takes one of three forms. “Conformity demonstrated” means the system satisfies all applicable requirements with no outstanding non-conformities. “Conformity demonstrated subject to remediation” means the system satisfies the requirements in substance, with outstanding major or minor non-conformities that have documented remediation plans and deadlines. “Conformity not demonstrated” means critical non-conformities remain unresolved, or the cumulative weight of major non-conformities is such that the Declaration of Conformity cannot be justified.

The Assessment Report is signed by the lead assessor and reviewed by the AI Governance Lead. It summarises the assessment scope, methodology, assessor team, findings by phase, the Non-Conformity Register summary, and the overall conclusion. The report is retained for the ten-year period as the evidential foundation for the Declaration of Conformity.
