Official Template Sources Seven artefacts have official or authoritative template sources as of February 2026. The European Commission published a draft serious incident reporting template on 26 September 2025, with the final version expected in August 2026. ECNL and the Danish Institute for Human Rights published a FRIA guide with a downloadable questionnaire template. Three further artefacts have their structure prescribed directly by the regulation’s annexes: Annex V (Declaration of Conformity, eight mandatory elements), Annex VIII (EU database registration, Sections A/B/C data fields), and Annex IV (AISDP documentation structure). The Commission’s own FRIA template under Article 27(5) had not been published as of February 2026. No other official fillable templates have been published by the Commission, AI Office, or any national competent authority for the remaining 54 artefacts.
