Contains eight mandatory elements per Annex V: system identification, provider identification, sole responsibility statement, conformity statement listing all applicable legislation, data protection compliance, standards and specifications applied, notified body information, and signatory with date. Signing carries personal liability; an inaccurate Declaration exposes the signatory and the organisation to Tier 3 penalties under Article 99(5). The Declaration can only be signed when the assessment report supports it. Responsible party: AI Governance Lead signs. Legal and Regulatory Advisor witnesses and confirms legal sufficiency.
